Instructions to Request a Consult With Dr. Julia Bridge

Required Documentation and Materials

  1. A cover letter that includes your request for review by Dr. Bridge and any requested ancillary testing
  2. Contact information for your facility
    1. This will be used in case we need to request more materials and to return materials.
  3. Pathology report
  4. H&E slide
    1. If H&E slide isn’t provided, we will cut and stain an H&E internally.
  5. Blocks or unstained slides
  6. Patient demographics
    1. If you have requested patient billing or if you are not an ARUP client, please include patient insurance information.

How To Order Electronically

  1. Order ARUP test code 2013263 through ARUP Connect™ or your LIS interface.
    1. Add a comment to the order that includes the following information:
      1. Your request for review by Dr. Julia Bridge
    2. Any ancillary test name(s) that are requested
      1. Note: Please do not place a separate order(s) for the additional testing.
  2. Provide the required documentation listed above and include any additional relevant documents from the patient’s medical record that are not mentioned.
  3. Securely pack material in padded packaging using either ARUP’s Paraffin Tissue and Extracted Nucleic Acid Transport Kit (preferred) or plastic slide holders. If you are shipping a paraffin block, include an ice or gel pack to ensure that the blocks do not melt in transit, especially during warmer months.
  4. Send materials to the shipping address listed in this document using your ARUP courier or your mail carrier (FedEx is preferred). Select 1–2 day shipping.

How To Order Manually

  1. Call 1-800-522-2787 and request requisition form #32960.
  2. Once obtained, fully complete the top portion of the form.
  3. On the requisition form, select Surgical Pathology Consultation, test code 2013263, and include the following:
    1. List the number of slides and blocks being submitted.
      1. You request for review by Dr. Julia Bridge
    2. Any ancillary test name(s) being requested
      1. Note: Please do not place a separate order(s) for the additional testing.
  4. Provide the required documentation listed above and include any additional relevant documents from the patient’s medical record that are not mentioned.

  5. If patient billing is requested, include patient demographic and insurance information.
