A leave of absence is available so you may care for you or your family, maintain your health, or recharge your batteries.
The Family Medical Leave Act (FMLA)
- Allows you to be away from work to care for a serious medical condition for yourself, parent, spouse or child
- Allows you to be away from work for Military reasons
- Allows you to be away from work to bond with your child
- Job protected
- Can be taken Intermittent, Continuously or Reduce your schedule
- This leave type is unpaid
What is FMLA
Family Medical Leave Act (FMLA)
The Family Medical Leave Act (FMLA) of 1993 is a law intended to help those who need to take an unpaid, job protected leave of up to 12 weeks during a 12-month period for specified family and medical reasons. Throughout your approved leave, FMLA will protect your job in your absence and allow for continued group health insurance coverage. A FMLA leave can be taken in a continuous block, intermittently or a reduced schedule, depending on what your provider thinks is best for you.
Eligibility for FMLA requires employees to have worked at ARUP for 12 months and also have worked 1250 hours. If you have not met this requirement, you may qualify for a Medical Leave.
Examples of why you may need FMLA:
- Inpatient care
- Incapacity relating to pregnancy or prenatal care (baby bonding)
- Incapacity for more than three days with continuing treatment by a health care provider
- Chronic serious health conditions
- Certain conditions requiring multiple treatments
- Military duties
How to Request
To begin a request for FMLA contact Lincoln Absence Management by phone:
- Call 1-888-244-7535
- Option 1 (you are an insured)
- Enter your Social Security Number
- After confirming social security number, select option 1, which will connect you to a representative that will assist with both the absence and short term disability claim.
What you will need:
- Social Security number
- The treating physician’s name and fax number
- Last date worked
- Condition or circumstances
- Direct deposit info for Short Term Disability payments
What you need to know:
- Please follow normal call in procedures for your department.
- A planned absence should be reported at least 30 days prior to the time off.
- An unplanned leave must be reported as soon as possible, according to company absence reporting practices.
- Intermittent time usage for approved leaves must be reported before your shift to your Supervisor and Lincoln Absence Management.
- Lincoln Absence Management will determine short term disability benefits.
- Pay is not issued until both the leave and short term disability are determined
- You can choose to use PTO during a determined leave, for unpaid portions
- Maximum combined leave time: 6 months in a rolling calendar year

Medical Leave of Absence (MLOA)
- Allows you to be away from work for your own medical condition
- Intended for those who don’t qualify for FMLA
- Your job is not protected and could be filled at any time during your absence
- Can be taken continuously or a reduced schedule
- This leave type is unpaid
What is Medical Leave
A Medical Leave of Absence (MLOA) is intended for those who do not qualify for FMLA or have exhausted their FMLA time. MLOA can only be used for your personal injury or illness, and is not job protected. MLOA can only be taken in a continuous block of time for up to 3-6 months depending on provider certification and length of employment at ARUP.
Examples of why you may need MLOA:
- You do not meet FMLA qualifications
- Inpatient care
- Incapacity for more than three days with continuing treatment by a health care provider
- Incapacity relating to pregnancy or prenatal care (baby bonding is not eligible for MLOA, bonding would fall under a Personal Leave
- Chronic serious health conditions
- Permanent or long-term incapacity
- Certain conditions requiring multiple treatments

- Enter your Social Security Number
- After confirming social security number, select option 1, which will connect you to a representative that will assist with both the absence and short term disability claim
- Please be advised that due to COVID-19, Lincoln’s phone lines are currently inundated with calls and wait times are consequently high. To avoid waiting on hold for a long period of time, please input your name and number into the virtual return call queue and they will call you back.

Personal Leave of Absence (PLOA)
- Can be taken for any reason
- Must be approved by your supervisor and group manager
- Your job is not protected and could be filled at any time during your absence
- Must be taken continuously
- This leave type is unpaid
What is a Personal Leave
ARUP may grant a discretionary, unpaid Personal Leave of Absence (PLOA) in order to manage your seniority date and insurance benefits during an extended time away from work not covered by other types of absence.
How to Request
Once you have received approval from your supervisor and group manager, contact Lincoln Absence Management by phone

- After confirming social security number, select option 1, which will connect you to a representative that will assist you.
- You will be asked your Social Security number and leave dates
What you need to know:
- Max personal leave time is 3 months
- All PTO will be exhausted before going unpaid
- A planned absence should be reported at least 30 days prior to the time off
- An unplanned leave must be reported as soon as possible, according to company absence reporting practices.
- Maximum combined leave time: 6 months in a rolling calendar year

Paid Family Leave and State Leaves
Some states offer a leave of absence. If you work in any of the following states, click on the state for more information.
Sick Days
- Missed work for up to 3 days
- Non serious health condition (ex: colds & flu)
- Serious conditions require a leave of absence
- Normal call in procedures apply
What if I am sick?
If you need to be out of work for a few days due to a non-serious, acute health condition, please follow normal call in procedures for your department.
Examples of non-serious health conditions:
- colds and flu
- earaches
- upset stomach and minor ulcers
- headaches (other than migraines)
- routine dental, orthodontic or periodontal disease
Would I get paid?
If you are out of work for a few days due to a non-serious, acute health condition, you could use PTO and any accrued grandfathered LTS. Grandfathered LTS was accrued by employees prior to 2019, to be used for their own medical condition.
Don’t have Grandfathered Long Term Sick?
If you were hired after 2019 or have exhausted your LTS bank, please work with your supervisor to enter PTO.
How to request the use of Grandfathered Long Term Sick?
- Fill out a Request for Long-Term Sick form
- Attach a health care providers note
- Submit both the form and note to HR (absence@aruplab.com)
What you need to know
- You cannot submit a request for long term sick to HR, if you are missing work due to surgery or a serious health condition. In these cases you will need to file for FMLA or MLOA.
- PTO will be used for the first 2 full shifts and if you have any grandfathered LTS it will be used beginning on the 3rd day missed. If you do not have PTO, the first 2 full shifts will be unpaid.
